The qualified Nurse Practitioner will collaborate on the development of our growing population health services and community partnerships.
The Opportunity Part-time position: 0.
5 FTE (two and a 1/2 days/week) This position will work primarily out of the LBH Mobile Van visiting schools and other community partners in Baltimore and Baltimore County to provide primary care services (i.
e.
, vaccinations, sports physicals, wellness exams, chronic disease management, etc.
) Will be accompanied by a clinical staff member.
Remote access to EMR system, laptop computer and printer No On-Call responsibilities.
No regular weekend or evening hours.
Identify patients needing primary care services and connect them with Grace Medical Center’s Ambulatory Services program for continued care.
Requirements Master’s degree and Advanced Practice Nurse, Nurse Practitioner license (APN, APRN, or NP) Demonstrated computer proficiency.
Excellent documentation and organizational skills.
Maryland DEA & CDS license.
Current CPR license.
Excellent interpersonal and human relations skills 1 – 2 years’ NP experience required Demonstrated proficiency in direct patient care Ability to work collaboratively with all members of the health care team, patients, and families Knowledge of regulatory and accrediting agency standards and requirements (i.
