Details Sign-on bonus: $3,500 Department: Care Management Schedule: Monday-Friday, 8:00am-4:30pm.

Agnes Location: Baltimore, MD The licensed Social Worker position with Care Management will support Maternal Health.
Maternal Child Health experience is supporting mothers and babies in the Baltimore city and surrounding counties to support SDOH and SBIRT positive support needs.
You will have a dynamic role with a student intern to support you.
Must be internally motivated to succeed, enjoys creating programs and has strong clinical judgement.
Excellent communication skills and professional in both behavior and performance, and experience with patient facing case management for medical needs.

Responsibilities Apply specialized clinical knowledge and advanced clinical skills in the areas of assessment, diagnosis, and treatment of mental, emotional, behavioral and developmental disorders, conditions and addictions.
Administer and interpret tests and measures of psychosocial functioning for the purpose of diagnosing mental, emotional, behavioral, addictive and developmental disorders and disabilities.
Develop and implement appropriate assessment-based treatment plans.
Consult with team manager, statutory and voluntary agencies and patient's relatives to ensure compliance with the patient's treatment plan.
Provide crisis-oriented psychotherapy as well as short-term and long-term psychotherapy and psychotherapeutic treatment.
Document patient therapy in accordance with both federal and state regulations.
Requirements Licensure / Certification / Registration: One or more of the following required: Social Worker credentialed from the Maryland Board of Social Work Examiners obtained prior to hire date or job transfer date.
Any level accepted.
Social Worker specializing in Clinical Practice credentialed from the Maryland Board of Social Work Examiners obtained prior to hire date or job transfer date.
Any level accepted.
Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences LMSW with at least 1 year of experience in Maternal Child Health to work with the Maternal child health grant.
Masters of Social Work - LCSW / LMSW or LCSW-C.
Required to have license.
The Social Worker is required to be Onsite.
Experience with Maternal Health (Mother/Baby), willing to train Must be able to work independently.
