JOB TITLE: Part Time Resident Service Coordinator
REPORTS TO: Community Manager
FLSA STATUS: Non-Exempt
POSITION SUMMARY: The Part Time Resident Services Coordinator (RSC) is a service manager to residents.
They may provide links to outside community agencies and negotiate affordable services, as needed.
The RSC educates residents on available services, develops contacts with providers and agencies for resident referrals and monitors the quality and quantity of delivered services.
They also provide informal counseling, information, and referrals, plan educational programs and coordinate volunteer opportunities.
The Resident Service Coordinator works in conjunction with the Community Manager(s) and other management staff to empower residents to positively age in place and remain as independent and self-reliant as possible.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following but are not limited to:
Case Management
Provides general assistance related to supportive, social services and activities designed to promote safe independent living.
Refers residents to services providers who can meet their needs, assist residents with accessing community-based services, monitor the receipt and follow-through of services, including encouraging and motivating residents to engage with providers and participate in their own services management.
Develops and maintains up-to-date resource and service provider directory for easy referrals.
Reports all suspected abuse situations to the appropriate agency.
Engagement & Outreach
Facilitates, proactively develops and arranges educational and preventative health programs, screenings and events
Facilitates the formation of a Resident Association, as needed.
Cultivates relationships with the Area Agency on Aging, senior centers, and other community-based resource providers
Networks with professional organizations and advocacy groups to learn and share trends and best practices
Assists the residents in building informal support networks with other residents, family and friends.
Advocacy
Act as a liaison with management or community agencies and often advocate on resident's behalf in groups, one to one, formally or informally
Helps residents with individual forms such as utility, phone, medical services; may fill out forms that they cannot fill out themselves; arrange utilities, phone, medical, and other payment schedules;
Addresses questions related to Social Security earnings, .
connect with hospice and bereavement counseling or supportive services; and solve other "bureaucratic" problems.

Assist residents with obtaining assistive technology and mobility devices
Assessment & Administrative
Conduct comprehensive, non-clinical assessments of resident wellness and social needs
Document resident interactions, maintain individual resident files, and prepare reports regarding service provision and update service plan in accordance with governing bodies and population needs.
Interpret and analyze assessment data to develop service plans
Does not perform duties or functions that are assigned to management or are associated with management responsibilities.

SKILLS/QUALIFICATIONS:
Bachelor’s degree is preferred but not required, or appropriate work experience
Two to three years of experience in social service delivery
Demonstrated working knowledge of supportive services and the aging process
Ability to advocate, organize, problem-solve and innovate
Must enjoy resident interactions and have the ability to prioritize multiple demands and meet deadlines required.
Ability to work effectively with diverse populations of resident and team members including age, economic status, interests and cultural background.
Proficiency with Microsoft Office Suite preferred.
